This is a good point in our discussion to describe a very common condition, which can be easily confused with RA or SLE. The syndrome of fibromyalgia is generalized noninflammatory rheumatism. Patients complain of generalized pain, as much in the muscles as in the joints, they sleep poorly, and feel un rested in the morning, and fatigued throughout the day. Forty percent state that they have swollen joints. Interestingly, there are several other associated syndromes such as migraine headaches, TMJ, costochondritis, MVP, IBS, PMS, and irritable bladder, to name a few. These patients are not malingering. Their symptoms stem from a neurotransmitter disturbance, and are quite convincing. The image shows various specialists dealing with these syndromes, quite possibly in the same patient. Unnecessary studies, and dangerous treatments that are not indicated are frequently ordered. But on the physical exam, you will find only characteristic tender points. There will be no signs of inflammation, or other evidence for immune attack. Lab studies are all normal. |
