This picture shows a hand with a disorder that we haven't seen yet. This disease is not uncommon and it will be encountered in a primary care clinic, but it is often much more subtle than this and it is therefore overlooked. This patient's toe is shown. The pattern of arthritis is quite different from what we've seen. In fact, this lesion appears to be a deposit outside of the joint. Note the off-yellow color of the material in the lesion which is very characteristic. This finger is from a different patient. The deposit is more extensive. This is tophaceous gout and these are tophi. Another topheous is shown. In this gentleman, nearly his entire foot was comprised of a giant tophous. His hands are shown. He had been misdiagnosed as having rheumatoid arthritis for 15 years. |
