As I have stated above, the pattern of joint involvement is very useful in the differential diagnosis of arthritis. To review, in OA the CMC, PIP and DIP are frequently involved, whereas the writs and MCPs are not. Other involved areas include the cervical and lumbar spine, hip, knee, and 1st MTP or bunion joint. A bunion is a deformity of the 1st MTP, resulting from OA in that joint. A bunionette, or Tailor's bunion of the 5th MTP is shown. Other rarely involved joints include the shoulder, elbow, and ankle. In RA, frequently involved joints include the wrist, MCPs and PIPs, whereas the CMC and DIP joints are spared. Other involved joints include the cervical spine, shoulder, elbow, hip, knee, ankle, and all the MTPs. |
